Note from the designer: "The ONA reports were usually printed at the end of a hectic annual report season, after which I promptly flew to Bermuda with my family to recuperate. I had hardly settled down with a gin and tonic when I received a call from my office with the message that ONA's chairman wanted a different cover for the report. In the photograph it seemed as though the elderly lady was giving a tip to the airline stewardess.

We scrambled for a different photo and came up with the "two pilots" cover. The entire report had to be reprinted as the earlier one had already been printed and bound. Actually, the chairman didn't much like the new cover either, because of the so called "fifty mission crush" in the pilot's cap, a WWII reference to removing the wire from stiffened caps after having completed a certain number of combat missions."
